ABSTRACT

Objective:To analyze the efficacy of dienogest combined with traditional Chinese medicine(TCM) in the treatment of endometriosis (EMs) and its influence on pregnancy and recurrence.Methods:A total of177 patients with EMs admitted to Affiliated Hospital of Shandong University of Chinese Medicine and Linyi Hospital of Traditional Chinese Medicine from December 2018 to June 2019 were divided into the observation group (88 cases) and the control group (89 cases) according to the random number table method. The control group was treated with dienogest 25 mg once a day, and the observation group was treated with dienogest combined with TCM decoction. They all were treated for 6 months. The clinical efficacy, pregnancy and recurrence of the two groups were compared, and EMs health profile-5 (EHP- 5) of the Chinese version was used to evaluate patients′ quality of life of the two groups.Results:The total effective rate in the observation group was higher than that in the control group: 92.04%(81/88) vs. 79.78%(71/89), and the difference was statistically significant ( χ2 = 5.492, P<0.05). The time of ovulation recovery and menstrual recovery in the observation group were shorter than those in the control group: (13.38 ± 3.60) d vs. (14.69 ± 3.15) d, (26.48 ± 4.23) d vs. (28.72 ± 3.40) d, and the differences were statistically significant ( P<0.05). There was no statistically significant difference in the 1-year pregnancy rate between the two groups ( P>0.05). The 1-year recurrence rate in the observation group was lower than in the control group: 1.14%(1/88) vs. 10.11%(9/89), and the difference was statistically significant ( P<0.05). The scores of pain, emotional state, sense of control, sexual life, doctor-patient relationship , infertility, and treatment effect in EHP-5 in the observation group after the treatment were lower than those in the control group, and the differences were statistically significant ( P<0.05). Conclusions:Compared with dienogest treatment alone, the combination of TCM treatment can further improve the efficacy. It is beneficial to pregnancy. It can reduce the recurrence rate, improve the quality of life more significantly, and has great safety.

OBJECTIVE@#To evaluate the accuracy and safety of pedicle screw placement using a new self-guided pedicle tap.@*METHODS@#According to the anatomical characteristics of the pedicle, a new self-guided pedicle tap was developed. Six adult spinal specimens including 4 males and 2 females were selected and tapped thread on the right and left sides of each pair of pedicles from the same segment T to L with traditional taps (control group) and new self-guided pedicle taps (experimental group), respectively. And the pedicle screws were placed. The screwing time was recorded and compared between two groups. CT scanning was completed to observe the accuracy and safety of the screw placement according to the Heary classification of imaging results.@*RESULTS@#The screwing time of thoracic and lumbar vertebrae in the experimental group were (5.87±1.25) min and(5.45±1.67) min, respectively. While those in the control group were (6.12±1.69) min and (6.22±2.13) min, respectively. Then there was no significant difference in screwing time of thoracic and lumbar vertebrae between two groups (>0.05). The Heary grade of the pedicle screw showed that Heary gradeⅠand Heary gradeⅠ+Ⅱwere respectively 86 (84.31%) and 96 (94.12%) in the experimental group, 72 (71.29%) and 84 (83.17%) in control group, and the difference between two groups was statistically significant (<0.05).@*CONCLUSION@#The new self-guided pedicle tap can accurately and safely insert the thoracolumbar pedicle screw, with low cost, easy operation, and good clinical application value.

ABSTRACT

OBJECTIVE@#To evaluate the therapeutic effects and bone cement distribution of unilateral percutaneous kyphoplasty (PKP) with oriented bone cement injector for thoracolumbar osteoporotic vertebral compression fractures (OVCFs).@*METHODS@#The clinical data of 211 patients (211 vertebrae) with thoracolumbar osteoporotic compression fractures underwent PKP between July 2016 to July 2018 were retrospectively analyzed. All punctures were performed unilaterally:102 patients in observation group used oriented bone cement injector including 30 males and 72 females with an average age of (68.4±8.9)years old; 109 patients in regular group used traditional bone cement injector including 32 males and 77 females with an average age of (70.4±9.2) years old. The two groups were compared in terms of duration of operation, cement volume, visual analogue scale(VAS), Oswestry Disability Index(ODI), distribution of bone cement, bone cement leakage and Cobb angle modified rate.@*RESULTS@#There were no significant difference in gender, age and fracture vertebra between the two groups(>0.05). No significant difference was found between two groups in duration of operation, cement volume, VAS, ODI and Cobb angle(>0.05). In observation group, 10 cases occurred cement leakages, with leakage rate of 9.80%; and in regular group, 11 cases occurred cement leakage, with leakage rate of 10.09%. There was no significant difference in the cement leakage rate between two groups(>0.05). In observation group, proportion rate of I-IV degree in cement distribution was 60.78%, 19.61%, 9.80%, 9.80%, respectively; while 39.45%, 22.93%, 22.93%, 14.68% in regular group. The I degree of cement cross-filling rate was better in observation group than in regular group(0.05). From T10 to L₅, I degree bone cement distribution rate of both groups showed decline trend. The I degree cement cross-filling rate in L₁-L₅ was 50% in observation group and 30.23% in regular group(<0.05).@*CONCLUSIONS@#Oriented bone cement injector can control the direction of bone cement dispersion and achieve effective distribution of bilateral bone cement using unilateral puncture and satisfactory surgical results. It is feasible and effective for unilateral PKP treatment of OVCFs.

ABSTRACT

Heparin-induced thrombocytopenia (HIT) is a relatively infrequent complication of heparin administration. HIT can cause devastating thrombosis, making it one of the most serious adverse drug reactions encountered in clinical practice. We successfully treated a case of severe HIT presenting with thrombosis and life-threatening bleeding complications with intravenous immunoglobulin (IVIG), platelet transfusion and oral anticoagulant Rivaroxaban. In this case, we considered that IVIG played the most important role by preventing further thrombosis, increasing the platelet count, and ensuring the efficacy of Rivaroxaban. We therefore suggest that IVIG might be the optimal treatment for patients with this urgent condition.

ABSTRACT

As the most important nuclear transcription factors in the cells, NF-κB is involved in many intracellular signaling pathways and transcription and regulation of genetic information. The signal transduction pathways mainly include the activation of IκB kinase, degradation of IκB protein and the nuclear translocation of p65. p65 trans-nuclear binding with DNA is the key for NF-κB to play a role. Abnormal activation of NF-κB is a major factor in the induction of oxidative stress, inflammation, cancer and so on. Therefore, maintaining the balance of NF-κB activity and regulating the nuclear translocation of p65 have great significance for further research on related subjects. In this paper, the regulation effects of the main active substances of medicinal plants (such as polyphenols, saponins, and alkaloids) on p65 nuclear translocation and the upstream pathway of NF-κB were discussed, expecting to provide reference for the development of natural active substances for functional food.

ABSTRACT

Purpose The aim of this study was to investigate the relationship between autophagy gene Beclin1 and immune response effector classical HLA Ⅰ,Ⅱ in SKOV3 cells.To explore the role of Beclin1 in immunity in ovarian cancer cells which were transfected with the vector of Beclin1.Methods RT-PCR and Western blot were used to detect the expression of Beclin1 and HLA Ⅰ,Ⅱ in SKOV3 cells.Fluorescence microscope was carried out to observe the unique autophagosome in SKOV3 cells.MTT was used to analyze the proliferation of the Beclin1 over-expressed SKOV3 cells.Results Transfection SKOV3 cells with Beclin1 vector could induce Beclin1 transcription and translation approximately 5 and 2 times compared with empty vector group respectively.The autophagosome stained by MDC was observed by fluorescence microscope.And much more green fluorescence signal was observed in Beclin1 vector group.RT-PCR and Western blot indicated that HLA Ⅰ,Ⅱ induced by transfection with extrinsic Beclin1.The allelic transcriptions of HLA Ⅰ-A,B,C and HLA Ⅱ-DP,DQ,DR in extrinsic Beclin1 group were approximately 2,1.6,3 and 2,6,3 times compared with empty vetcor group or untreated group,respectively.The results of Western blot showed that HLA Ⅰ,Ⅱ in Beclin1 vector group induced as much as 2 and 1.6 times compared with empty vetcor group or untreated group,respectively.The results of MTT showed that the proliferation of SKOV3 cells treated with Beclin1 vector was significantly suppressed.The percentage of suppression in Beclin1 vector group at 24 h,48 h,72 h and 96 his42.6%,37.8%,24.35%,14.81% compared with untreated group or empty vector group respectively.Conclusion The enhancement of autophagy by over-expression of Beclin1 could induce HLA Ⅰ,Ⅱ transcription and translation in SKOV3 cells.The expression of HLA Ⅰ,Ⅱ may be responsible for triggering the immune response in ovarian cancer.Over-expression of Beclin1 could inhibit the proliferation of SKOV3 cells which were transfected with extrinsic Beclin1.

ABSTRACT

Objective Hyperglycaemia and hyperlipid are common in clinical cardiovascular disease .The article was to ob-serve the change of endothelial and the expression of Rho-kinase in the aorta of rats with hyperglycaemia and hyperlipid and the rela-tionship between them .Methods Healthy male Wistar rats were randomly divided into 3 groups:5 as normal control group , 10 as model group and 10 as drug intervention group .The latter two groups were given high-fat diets for 5 weeks after tail vein injection of streptozocin (STZ) to establish hyperglycaemia and hyperlipid models , then fasudil (inhibitor of Rho-kinases) or saline were respec-tively given intraperitoneally for 4 weeks.Venous blood glucose (VBG), blood lipid levels, ET-1 and eNOS were tested, along with the expression of MYPT-1(the protein substrates of ROCK)、mRNA of RhoA and ROCK. Results Compared with normal control group, the levels of VBG、ET-1、MYPT-1、mRNA of ROCK and Rho A were increased in model group[(6.18±1.14)mmol/L vs (26.36± 3.25)mmol/L,(72.13±6.13 )pg/mL vs (88.22±4.61) pg/mL, (0.54±0.08) v s (1.28±0.17),(0.16±0.20) vs (0.83± 0.12),(0.40±0.18) vs (0.78±0.13),P<0.05].TG、TC、LDL were also higher in model group(P<0.05), while eNOS level was lower in model group than in control group[(24.42±2.13)U/L sv (17.36±1.58)U/L,P<0.05].Compared with model group, the levels of VBG[(17.70±2.69)mmol/L]、TG、TC、LDL, ET-1 [(75.03±2.50)pg/mL]、MYPT-1(0.74±0.01)、mRNA of ROCK(04.0±0.08) and Rho A (0.25±0.07)in drug intervention group were lower(P<0.05), while eNOS level (0.74±0.11) was higher(P<0.05) in drug intervention group than in model group . Conclusion Endothelial dysfunction and over-expression of Rho kinase can be found in hyperglycaemia and hyperlipid rat models .Fa-sudil could improve endothelial function .

Objective: To study the chemical constituents in the dry mature seeds of Myristica fragrans (Myristicae Semen). Methods: Compounds were isolated and purified from the ethyl acetate layer of 75% alcohol extract by means of the silica gel column chromatography, Sephadex LH-20 column chromatography, and preparative HPLC isolation methods. Compound structures were identified by analyzing and comparing the spectral data with those for them in the reference. Results: Eighteen compounds were obtained from the ethyl acetate layer of 75% ethanol extract in Myristicae Semen. On the basis of spectral data and combined with the references, these compounds had been identifited as: vanilic acid (1), butein (2), (2R)-3-(3', 4', 5'-trimethoxyphenyl)-1, 2- propanediol (3), sulphuretin (4), 3-methoxy-4, 5-methylenedioxy-cinnamic acid (5), 7, 3', 4'-trihydroxyflavanone (6), 7-hydroxy- 4-benzopyrone (7), verrucosin (8), (+)-erythro-(7S, 8R)-Δ8'-7-hydroxy-3, 4, 3', 5'-tetramethoxy-8-O-4'-neolignan (9), (-)-erythro-(7R, 8S)-Δ8'-7- acetoxy-3, 4, 3', 5'-tetramethoxy-8-O-4'-neolignan (10), nectandrin B (11), (-)-(7S, 7'R, 8S, 8'R)-4, 4'-dihydroxy-3, 5, 3'-trimethoxy-7, 7'-epoxylignan (12), fragransin B3 (13), fragransin B1 (14), (-)-enantiomer (15), (-)-erythro-(7R, 8S)-Δ8'-7-hydroxy-3, 4, 5, 3', 5'-pentamethoxy-8-O-4'-neolignan (16), (+)-erythro-(7S, 8R)-Δ8'-7, 4-dihydroxy-3, 5, 3', 5'-tetramethoxy-8-O-4'-neolignan (17), and (+)-5-methoxydeydrodiisoeugenol (18). Conclusion: Compounds 2 and 4-7 are isolated from the plants of Myristica Gronov. for the first time. Compound 1 is isolated from this plant for the first time.

ABSTRACT

Objective:To explore the relationship between wearing of functional appliance and systemic metabolism of PGE2 in the blood of growing rats.Methods:54 male Wistar rats aged 4-weeks were randomly divided into 3 groups(n =18):Daytime group,all-day group and control group.Self-guided functional appliance was made for mandible advancement of the rats in the 2 experimental groups.One week after appliance wearing,rats were sacrificed at the corresponding time points and blood in femoral artery was taken. PGE2 levels in the blood samples were measured,data were analysed by cosine equation.Results:Hypothesis was true in the control group(P <0.05),and was not true in both 2 experimental groups(P >0.05).Conclusion:PGE2 level in rat blood is circadian rhyth-mic,wearing of functional appliance made may disturb the rhythm.

ABSTRACT

Human papillomavirus (HPV)-induced cervical cancer is the second most common cancer among women worldwide. Despite the encouraging development of the preventive vaccine for HPV, a vaccine for both prevention and therapy or pre-cancerous lesions remains in high priority. Thus far, most of the HPV therapeutic vaccines are focused on HPV E6 and E7 oncogene. However these vaccines could not completely eradicate the lesions. Recently, HPV E5, which is considered as an oncogene, is getting more and more attention. In this study, we predicted the epitopes of HPV16 E5 by bioinformatics as candidate peptide, then, evaluated the efficacy and chose an effective one to do the further test. To evaluate the effect of vaccine, rTC-1 (TC-1 cells infected by rAAV-HPV16E5) served as cell tumor model and rTC-1 loading mice as an ectopic tumor model. We prepared vaccine by muscle injection. The vaccine effects were determined by evaluating the function of tumor-specific T cells by cell proliferation assay and ELISPOT, calculating the tumor volume in mice and estimating the survival time of mice. Our in vitro and in vivo studies revealed that injection of E5 peptide+CpG resulted in strong cell-mediated immunity (CMI) and protected mice from tumor growth, meanwhile, prolonged the survival time after tumor cell loading. This study provides new insights into HPV16 E5 as a possible target on the therapeutic strategies about cervical cancer.

<p><b>OBJECTIVE</b>To study the association of FCGR2A gene single nucleotide polymorphism (SNP) rs1801274 with Kawasaki disease (KD) susceptibility and the efficacy of intravenous immunoglobulin (IVIG) therapy in Han Chinese children.</p><p><b>METHODS</b>Thirty-five KD children and 25 age-and gender-matched healthy children (control group) were enrolled in the study. Polymerase chain reaction (PCR) and gene sequence analysis were applied to detect SNP of FCGR2A gene rs1801274. These KD patients were classified into two subgroups based on the presence of coronary artery lesion (CAL) following IVIG therapy: CAL (n=13) and non-CAL (n=22).</p><p><b>RESULTS</b>FCGR2A gene SNP rs1801274 was detected in all subjects, including three genotypes (AA, AG and GG). For FCGR2A gene SNP rs1801274, there were significant differences in the genotype and allele frequencies between the KD and control groups (P<0.05), and significant differences in the genotype and allele frequencies were also found between the CAL and non-CAL subgroups (P<0.05). A allele and AA genotype were linked to an increased risk of KD susceptibility (A allele: OR=3.39, 95%CI:1.53-7.50; AA genotype: (OR=4.93, 95%CI:1.61-15.1). Both AG (OR=5.43, 95%CI:1.06-27.8) and G allele (OR=4.88, 95%CI:1.44-16.5) were linked to an increased risk of CAL in KD children.</p><p><b>CONCLUSIONS</b>Polymorphism of the FCGR2A gene SNP rs1801274 is one of the important factors probably influencing susceptibility to KD and efficacy of IVIG therapy on KD in Han Chinese children.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To discuss the treatment of congenital hypospadias in infancy.</p><p><b>METHODS</b>After correction of chordee, the tubed oral mucosa was used to prefabricate urethra in penis as free graft. The urethral anastomosis was performed at the second stage. The scrotum fascia flap, pedicled with scrotal artery, was transferred to cover the penile defect.</p><p><b>RESULTS</b>From Jan. 2007 to May 2010, 42 cases were treated. The tubed oral mucosa was 3.0-4.0 cm in length, and 0.6-0.8 cm in diameter. The maximum size of scrotum flap was 1.5 cm x 3.0 cm. Urethral fistula happened in two cases due to necrosis at the distal end of scrotal flap, which was healed spontaneously after dressing for 1-2 weeks. Primary healing was achieved in all the other cases. After operation, the 26 cases have been visited for 6 months to 3 years, no urethra fistula , narrow and the penis curved. There is an universal urinate for oneself and a good form.</p><p><b>CONCLUSIONS</b>The tubed oral mucosa can be used to prefabricate urethra, which is anastomosed at the second stage. The tubed oral mucosa combined with scrotal fascial flap which has a reliable blood supply, is very suitable for hypospadias in infancy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the pathological characters and anatomic correction of penile epispadias.</p><p><b>METHODS</b>The urethra was formed by local urethra plate mucosa flaps. The contracture on dorsum of penis was released by cutting off the superficial suspensory ligament to reposition the penile and urethral sponge.</p><p><b>RESULTS</b>From Jun. 2004 to Dec. 2010, 26 cases with penile epispadias were treated. 18 cases were followed up for 6 months to 5 years. 10 cases were treated successfully with good cosmetic and functional results. Urinary incontinence happened in 8 cases, which healed after tightening operation.</p><p><b>CONCLUSIONS</b>The anatomic correction of penile epispadias can recover the normal anatomic structure, resulting good cosmetic and functional results.</p>

ABSTRACT

<p><b>BACKGROUND</b>The capsule associated protein 10 gene (cap10) is indispensible for the formation of the polysaccharide capsule, and is important in maintaining virulence of the Cryptococcus (C.) neoformans. In this study, we aimed to construct an short hairpin RNA (shRNA) expression vector targeting C. neoformans cap10 gene expression and confirm its biologic relevance.</p><p><b>METHODS</b>A pair of oligonucleotides targeting the cap10 cDNA sequence was designed and synthesized. It was cloned into the plasmid psilencer4.1-CMV neo to construct an eukaryotic shRNA expression vector. The vector was transfected into C. neoformans cells using the LiAc method. The expression of cap10 was assessed by real-time fluorescence quantitative PCR. Groups of C. neoformans cells were incubated with murine macrophage-like J774A.1 cells, and the phagocytic indexes and ratios were determined by the microscopic observation method.</p><p><b>RESULTS</b>The expression of cap10 in C. neoformans cells transfected with ps4.1 neo-cap10 ((175,535.00 ± 47,004.00) copies/µl) was lower than that of cells transfected with the empty vector ((512,698.89 ± 32,318.02) copies/µl) and mock transfected cells ((562,931.66 ± 65,928.41) copies/µl). The average phagocytic ratio and phagocytic index of J774A.1 cells following incubation with C. neoformans were higher for cells transfected with ps4.1 neo-cap10 (0.21 ± 0.02, (19.06 ± 1.66)%) than for the control experimental group (0.08 ± 0.02, (6.57 ± 1.23)%) and the blank experimental group ((0.07 ± 0.01), (5.89 ± 1.07)%) (P < 0.05).</p><p><b>CONCLUSIONS</b>The cap10 shRNA vector was successfully prepared and transfected into C. neoformans cells. The effect of RNA interference on the expression of the C. neoformans cap10 gene is effective, and it can induce phagocytosis of C. neoformans.</p>

ABSTRACT

Briefly introduce the development of acupuncture in Ecuador, including acupuncture development history for more than 20 years. In Ecuador, acupuncture department of public hospital and acupuncture clinic engage in clinic, and acupuncture has already been included in medical insurance. The acupuncture education mainly relys on training abroad, and only one university set up acupuncture subject at home. Local acupuncture management is non-standard, without high clinical level and short of regular and systematic acupuncture education system. However, Ecuador government pays important attention to development of acupuncture, and acupuncture is warmly welcomed by the folk, which can treat diseases in wide range in local area.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the biological function of platelet-derived growth factor B (PDGF-B) on the survival and proliferation of cat corneal endothelial cells so as to provide bases for further studies of its role in wound repair and its clinical application.</p><p><b>METHODS</b>Total RNA was extracted from the placenta tissues of healthy pregnant women undergoing hysterotokotomy and PDGF cDNA was obtained with reverse transcription-polymerase chain reaction (RT-PCR). The prokaryotic expression vector pET-PDGF-B was constructed and expressed the recombinant PDGF-B in Escherichia coli (E. coli) BL21 (DE3). After purification and refolding on Ni2+-chelation affinity chromatography (NTA) column, it was used to culture cat corneal endothelial cells. Cell proliferation was tested by modified tertrazolium salt (MTT) and flow cytometer. And the morphologic change and the ultrastructure were observed under an inverted phase contrast microscope, a scanning electron microscope and a transmission electon microscope, respectively.</p><p><b>RESULTS</b>PDGF-B chain peptide (PDGF-BB) gene was successfully inserted into the prokaryotic expression vector, pET-28a (+). The purified recombined protein pET-PDGF-B showed a single band on sodium dodecyl sulfate polyacrylamide gel electropheresis (SDS-PAGE) with the molecular weight of about 27 u, which was in agreement with the deduced value. MTT and flow cytometry showed that PDGF-BB promoted the survival and proliferation of cat corneal endothelial cells.</p><p><b>CONCLUSIONS</b>The construction of recombinant prokaryotic expression vector pET-PDGF-B and the preparation of PDGF-BB protein provide a foundation for further study of the function of PDGF-BB and producing biological PDGF-BB protein. The expressed PDGF-BB promotes the proliferation of cultured cat corneal endothelial cells.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To report the treatment of serious hypospadias in adults with free graft of tubed mouth mucosa and scrotal fascia flaps.</p><p><b>METHODS</b>The tubed mouth mucosa was free grafted to fabricate the distal segment of urethra. It was anastomosed to the urethra at the second stage. The scrotal fascia flap was used to cover the penile wound. The biggest flap was 3 cm in width and 6.5 cm in length.</p><p><b>RESULTS</b>From Jan. 2002 to Dec. 2007, 76 adults with severe hypospadias were treated. Infection happened in 4 cases. 2 cases had urethral fistula due to the partial flap necrosis which was healed automatically within 2-4 weeks. All the other patients healed primarily.</p><p><b>CONCLUSIONS</b>It is a good method for the treatment of serious hypospadias in adults with scrotal fascia flaps and free graft of tubed mouth mucosa which is anastomosed to the urethra at the second stage.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare therapeutic effects of fire needle therapy and filiform needle therapy on facial spasm.</p><p><b>METHODS</b>One hundred and twenty-seven cases of such disease were divided into two groups. The treatment group (n=79) were treated with fire needle therapy at Taiyang (EX-HN 5), Cuanzhu (BL 2) and Quanliao (SI 18), and the control group (n=48) with filiform needle therapy at the same acupoints as those in the treatment group. Their therapeutic effects were compared.</p><p><b>RESULTS</b>The cured rate and the effective rate were 64.6% and 92.4% in the treatment group, and 45.8% and 87.5% in the control group, respectively, with a significant difference between the two groups (P < 0.05).</p><p><b>CONCLUSION</b>Fire needle therapy is an effective therapy for facial spasm, with a therapeutic effect better than that of filiform needle therapy.</p>
